Do we need to create a custom post type template and add html to the top to display the header section correctly?
there is a way in Divi for me to set a template for the Custom Post Type, what is the equivalent of [discussion_board_form] for displaying a Topic/Question?

Regarding header, it seems to have a conflict with WPDB plugin and your active themes. We will need to troubleshoot further.
